The electronic health questionnaire does not work
Blood donors' electronic health questionnaire is temporarily out of service due to a technical problem. We are working to resolve the problem. At the moment blood donors fill out a paper questionnaire at the donation session.
The blood donors’ electronic health questionnaire does not open due to a technical problem. Please try again in a few moments. You can also fill out the questionnaire a short while before donating, or at the donation session.
At the moment it is possible for blood donors to fill out a paper questionnaire at the donation session, so you are welcome to come to donate blood normally if you have booked an appointment!
