Malfunction in electronic health questionnaire and mobile application
The electronic health questionnaire and mobile application for blood donors are currently unavailable due to a disruption in the information system. The malfunction is being fixed urgently. We apologize for any inconvenience caused to blood donors.

You can still come to donate blood, but we will use paper forms during the malfunction. You can fill in the health survey at the blood donation center now that the electronic questionnaire is not working. Please also be prepared for delays at the donation centers, as the use of paper forms will slow down the donation process.
If it has been more than five years since your last donation, we will unfortunately not be able to accept your donation during the system malfunction unless you have the mobile app or an old-style blood type card showing your blood type with you.
Always remember to bring photo ID with you to the donation, as blood donation always requires identity verification. We also accept driver’s licenses as valid ID.
